Student Absence Form

Prompt and regular attendance at all scheduled Upward Bound events is required by all Upward Bound participants. It is the participants’ responsibility to notify Upward Bound of an absence.

Please note that after three absences, participants will be required to meet with Upward Bound staff to discuss program participation. Additional absences may change a participant’s active status to conditional and continuation in the program could be in jeopardy.
